چکیده

Electrosurgical vessel sealing is used throughout many surgical procedures with the benefits of the device widely reported. However, there is still significant variation in the quality of the resulting seal with limited understanding as to why. This study developed a methodology to use digital image correlation to capture the sealing process and to investigate the changes occurring throughout. Porcine carotid arteries were used throughout the study, with seals created using the Gyrus G400 generator and the PKSOpen Seal device (Gyrus Medical Ltd, Cardiff, United Kingdom). The displacement across the surface of the blood vessel and the device jaws was measured, and the true maximum principal strain was computed. There was significant contraction of the tissue along the length of the blood vessel, with this occurring in pulses due to the waveform used to deliver the current. Additionally, there was a significant change in displacement between the device jaws (0.08mm), a novel finding of the study. This change in displacement is indicative of a change in application force throughout sealing, which can have significant implications in the quality of the seal, and therefore the findings of this study can influence future device design.

Rapid sealing and cutting of porcine blood vessels, ex vivo, using a high-power, 1470-nm diode laser.

Suture ligation with subsequent cutting of blood vessels to maintain hemostasis during surgery is time consuming and skill intensive. Energy-based electrosurgical and ultrasonic devices are often used to replace sutures and mechanical clips to provide rapid hemostasis and decrease surgery time. Some of these devices may create undesirably large collateral zones of thermal damage and tissue necr...

In silico evaluation of geometry variations with respect to the thermal spread during coagulation of egg white using bipolar vessel sealing instruments

BACKGROUND Bipolar vessel sealing is an efficient electrosurgical procedure for the occlusion of blood vessels particularly during minimally invasive surgery. Reliable knowledge of the thermal spread is crucial for a safe application of bipolar vessel sealing instruments when operating close to thermo-sensitive structures, such as nerves. The evolution of the thermal spread over time and space ...

Variation in electrosurgical vessel seal quality along the length of a porcine carotid artery.

Electrosurgical vessel sealing has been demonstrated to have benefits for both patients and practitioners, but significant variation in the strength of the seal continues to be a concern. This study aims to examine the variation in electrosurgical seal quality along the length of a porcine common carotid artery and explore the relationships between seal quality, vessel size and morphology. Addi...

Thermal spread of vessel-sealing devices evaluated in a clinically relevant in vitro model.

INTRODUCTION Bipolar vessel-sealing devices (VSDs) have advantages in urological surgeries (less hemorrhage, shorter operating time). However, these instruments can cause thermal injuries, which can result in neural damage and necrosis. The objectives of this study were to establish a reproducible in vitro model for standardized assessment of electrosurgical devices and to evaluate whether opti...
